HCHS Student Information Form Reminder

HCHS Families,

This is a friendly reminder to please complete and return your student's 2026–2027 Student Information Form if you have not already done so.

The bright yellow forms were sent home with students at the beginning of the school year. A completed form is required for each student every school year, even if your contact information has not changed.

Keeping this information current helps ensure that HCHS has the correct parent/guardian contact information, emergency contacts, address, and important student information on file when we need to reach you.

If you have already returned your student's form, thank you! If not, please complete it and have your student return it to HCHS as soon as possible.
